Name, address, email and phone number
Style and size of cup: 20 oz skinny, 30 oz skinny, 20 oz curve, 30 curve, Kids  12 oz curve, etc.
Color of cup/glitter
What you would like on the cup: Picture, saying, etc.
Name for the cup if adding
Is there a theme
As much info as possible
I will send you an estimate based on the information, $35 is starting point
We will create the cup together, payment is due prior to starting 
It will take 5-7 business days to ship, shipping 3-5 days. Local pick up available.
